At four years from first registration; it is exempt from periodic inspection before then and inspected every two years thereafter, under Article 6 of Royal Decree 920/2017. It does not depend on exceeding 125 cc. This scenario does not extend to every vehicle called a quad or to categories, uses or special regimes with different schedules.
